First Visit

We feel your initial appointment should be for a comprehensive oral exam. It is important that we have enough time to review your medical and dental history, check all of your teeth for decay and defective restorations, examine your mouth for oral cancer and periodontal disease, and take the necessary diagnostic x-rays. After Dr. Koza has diagnosed the results of your exam, he will discuss his findings and outline recommended treatment and the cost of treatment. It may be necessary to have you back for a consultation appointment at no additional cost to you. If you have had regular dental care, a dental cleaning may be incorporated with this appointment.

All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.), and bring a list of medications you are presently taking.

X-Rays

If a previous dentist has taken x-rays, you may request that they forward them to our office via email if possible. If the office is not able to email x-rays, they can be mailed or if there is not enough time, please pick them up and bring them to our office with you. If additional x-rays are necessary, they can be taken at our facility.  We utilize digital x-ray technology which greater reduces the amount exposure to x-rays.
